<p>A complain was written asking for close follow up for the chronic disease patients especially the DM.</p> <p>Prolong period of waiting until the next appoint for Central DM patients.</p> <p>Patient does not reciive the calls </p> <p>Informing the patient about his lab reports of HBA1C as normal but infact it was high.</p>
