Case Description <p>Dear Sir / Madam Today my daughter (Ms Aisha Mohammad Tabrez Mistry, CPR No 000814636) visited Shaikh Sabah Health Centre, Umm Al Hassam at 5.30 pm. The doctor confirmed that she is have viral fever and throat congestion. She told to download the Medical Leave Certificate from MoH website by entering the details. She is entering her all the details i.e. CPR number, Expiry date and Block number, but she is unable to download the Medical Leave Certificate. Therefore I am requesting your help and support for issuing the Medical Leave Certificate for my daughter Ms Aisha Mohammad Tabrez Mistry. I will much appreciate your cooperation in this regard.
