Birth & Death Procedures
- The Procedures and Documents needed to process Birth Certificate:-
- Parent's Passport
- Parent's CPR Card
- In case of the parents didn't inform, the informer should bring his/her passport or CPR Card.
- Copy of Notification of Birth form from the hospital or from home delivery midwife.
- The informer should sign after reading the form, since it is not possible to change any data after registration done without the Court permission.
- The Documents needed to process Death Certificate:-
- Passport and CPR Card for dead person.
- Passport or CPR Card for the informer.
- An Official Notification of Death form from the hospital or police after seen by public security doctor.
